Implementation plan for promoting the high-quality development of elderly care services in Shanghai
In order to thoroughly implement the opinions of the general office of the State Council on promoting the healthy development of elderly care services, this implementation plan is formulated in combination with reality.
1、 Work objectives
——The elderly care service system has been improved. By 2025, deepen the formation of an elderly care service system coordinated by home-based community institutions and combined with medical care and health care; Build a government led, family oriented, multi-party participation, and the combination of correctional and medical care service system; The overall promotion mechanism of "one old and one small" is more sound.
——The provision of elderly care services is more effective. By 2025, the number of nursing beds in the city will increase from 159000 to 178000, of which nursing beds will not be less than 60%; Community elderly care service facilities in all districts shall not be less than 40 square meters per 1000 permanent residents, and conditional districts shall strive to reach 50 square meters per 1000 permanent residents. There are 4.5 nurseries for infants and young children under the age of 3 per 1000 permanent residents in the city, with inclusive nurseries accounting for no less than 60%, kindergartens with nursery classes accounting for no less than 50% of the total number of kindergartens. Scientific childcare guidance services fully cover the families of school-age infants and young children of the permanent population.
——The quality of elderly care services has been significantly improved. 2、 Key tasks
（1） Promote the structural optimization and functional upgrading of elderly care services
1. Guarantee basic pension services. Formulate the list of basic elderly care services in Shanghai. Improve the construction and management methods of basic pension institutions (beds), clarify the construction, service standards and management requirements, and appropriately optimize the setting of housing types. Establish an overall utilization mechanism of basic pension beds, establish waiting rules and study incentive policies by relying on the pension service information platform, so as to facilitate the cross regional pension of the elderly. All districts are encouraged to establish an overall utilization mechanism of pension beds in their own regions to improve the overall utilization rate of beds.
2. Optimize the regional layout of elderly care service facilities. Formulate a special plan for the layout of elderly care service facilities in the 14th five year plan, and steadily increase elderly care beds and various community elderly care service facilities. Focus on new pension beds in new towns and densely populated areas for the elderly, and encourage central urban areas to further tap their potential and increase pension beds nearby. Research and implement the transformation and upgrading project of existing elderly care service facilities, improve software and hardware conditions and enhance professional service capacity.
3. Enhance the professional care function of elderly care institutions. Focus on the care needs of the disabled and mentally retarded elderly, and increase nursing beds and cognitive impairment care beds. Revise the guidelines for setting nursing beds in elderly care institutions, and increase nursing beds by means of new construction and transformation. Formulate local standards for the setting of cognitive impairment care beds, encourage the development of elderly cognitive impairment care institutions, and support the construction of cognitive impairment care units in elderly care institutions. By 2025, the number of care beds for cognitive impairment will reach 15000.
4. Classify and improve community embedded elderly care services. Promote the classified development of day care service institutions, introduce professional institution management, provide care, rehabilitation assistance and other services for some disabled and mentally retarded elderly, and bring qualified institutions and services into the coverage of long-term care insurance. Improve the check-in standard and exit mechanism of the elderly care home, and strengthen the short-term care function. Optimize the catering service for the elderly and encourage the participation of multiple subjects. Explore the development of family care beds. Carry out the construction of friendly communities with cognitive impairment and realize the full coverage of streets and towns. We will vigorously develop rural mutual care for the elderly and support the use of rural homesteads or idle collective construction land to build elderly care service facilities.
5. Deepen the combination of medical care and health care. Promote the establishment of community health service institutions and elderly care service institutions at the same address or adjacent to each other. Deepen the signing and cooperation service mechanism between elderly care institutions and medical institutions, and explore the way of projects to pay labor fees for medical staff who carry out integrated medical care services in elderly care institutions. Explore the establishment of regional medical and nursing consortia, and promote orderly referral and two-way referral among medical, elderly care, nursing, rehabilitation and other institutions. Promote the mode of "nursing home + Internet hospital" and provide services such as prescription, consultation, chronic disease management and health education for the elderly. Promote the combination of physical and mental health and build a sports and health home for the elderly. Promote the transformation of qualified training and convalescent institutions and develop elderly care services.
6. Improve the elderly health service system. Adhere to prevention first, popularize healthy lifestyles and promote the concept of healthy elderly care. Build a geriatric service network led by the municipal geriatric center, with the geriatric department of the general hospital as the backbone and based on community medical and health institutions. Encourage qualified medical institutions to transform into rehabilitation hospitals or nursing homes, and support the development of private nursing and rehabilitation medical institutions. Improve the hospice care service system and enhance the life dignity of dying patients. We will comprehensively carry out the construction of friendly medical institutions for the elderly, optimize the medical treatment process, and provide convenience for the elderly. We will provide family doctors with contracted services, improve the health records of the elderly, integrate information such as physical examination of the elderly, self inspection of intelligent health post stations, diagnosis and treatment records of medical institutions, and strengthen health management services for the elderly.
7. Deepen the pilot of long-term care insurance. Revise the pilot measures of Shanghai long term care insurance, and improve the policies of evaluation and identification, service content, process supervision, quality evaluation, payment standard and so on. Study and improve the payment standard of long-term care insurance for nursing services in elderly care institutions. Improve the third-party comprehensive evaluation standard system of designated service institutions of long-term insurance, and promote the healthy and standardized development. We will promote the pilot of extended settlement of long-term care insurance and elderly care institutions in the Yangtze River Delta, and support the elderly to provide elderly care in different places. Encourage commercial insurance institutions to develop commercial long-term insurance products, and explore the introduction of commercial insurance institutions to participate in long-term insurance handling services.
（2） Expand the effective supply of childcare services
1. Improve scientific parenting guidance services. Scientific parenting guidance will be included in the "one thing at birth", and the Shanghai maternal and child health manual will be distributed free of charge, so as to achieve full coverage of infant families of permanent residents, and provide pregnancy and health guidance from the beginning of pregnancy to childhood. Each street town is equipped with a family scientific parenting guidance station to provide guidance services for school-age infant families in need at least 10 times a year. Relying on the "bid along" mobile terminal, promote applications such as the "way of education" app, explore cooperation with internet platforms, and expand the awareness and coverage of free childcare resources. Carry out activities such as "parenting gas station" and "parent-child Carnival", and provide comprehensive services such as dietary nutrition guidance, growth and development testing, mental health and so on. Formulate and implement norms and evaluation methods for scientific parenting guidance, and strengthen quality supervision.
2. Promote the opening of nursery classes in kindergartens. In principle, new and additional classes should be opened in qualified public kindergartens, and classes should be opened in newly-built, reconstructed and expanded public kindergartens. Improve the management system of public kindergartens, and clarify the standards and norms of class hardware, teachers, enrollment and fund guarantee. Encourage qualified private kindergartens to set up inclusive classes.
3. Encourage all kinds of subjects to hold inclusive childcare institutions. We will improve the standards for the establishment of childcare service facilities and institutional management measures, ensure safety and service quality, and encourage more qualified social subjects to participate. Support qualified enterprises and institutions, parks, business buildings, etc., and use various resources to build or expand childcare service facilities to provide childcare services for employees.
4. Explore community education services. Formulate community care setting standards and management systems, encourage the use of various community comprehensive service facilities, build standardized and embedded "baby houses" and other care places, and provide universal care services such as temporary care and time care. Support the introduction of professional care institutions through public construction, private construction and purchase of services to standardize the management of community care places.
5. Promote the integrated development of "one old and one small". We will integrate elderly care services into the "15 minute community life circle" and the comprehensive service system at home, and strengthen the overall planning, co construction and sharing of resources. Provide venues, facilities and other facilities for the energetic elderly to take care of infants and young children in the community, and develop targeted parenting guidance courses to help alternative care. Give play to the role of trade unions, the Communist Youth League, women's federations and other mass organizations and public welfare social organizations to provide elderly care services for residents.
（3） Increase multi-level and diversified elderly care services

2. Promote the quality improvement and upgrading of elderly care products. Actively develop the industries of daily necessities for the elderly, nursing products, rehabilitation aids and other elderly products, and cultivate a number of "specialized and special new" small giant enterprises and single champion enterprises in the manufacturing industry. Support the holding of China international pension, assistive devices and rehabilitation medical Expo, and give full play to the function of industrial docking platform. Explore the inclusion of rehabilitation aids into the special channel for innovative approval of medical devices, speed up the approval process, broaden the application scenarios, and vigorously develop the characteristic industrial park of rehabilitation aids. 3. Promote the improvement and expansion of domestic services. Deepen the "leader" action of improving the quality and capacity of domestic services, and give play to the demonstration and driving role of the pilot area. 4. Accelerate digital transformation. We will implement the "digital Partnership Plan", carry out special actions to adapt Internet applications to aging and information accessibility, explore the use of market mechanisms to promote "one touch service for the elderly", strengthen digital skills education and training for the elderly, and help the elderly bridge the digital divide. （4） We will strengthen the construction of a contingent of elderly care and nursing talents
1. Strengthen the training of professional talents. Set up specialties or courses such as elderly service and management, infant care service and management in qualified colleges and universities and vocational colleges. Support Shanghai Open University to carry out academic education, non academic training and community education in the fields of elderly care, nursery education and housekeeping. Cultivate industry education integrated elderly care enterprises and support the construction of a number of industry education integrated training bases. Actively cultivate professional social workers in the field of elderly care.
2. Strengthen the skill training of employees. We will implement the project to improve the skills of elderly care workers, strengthen the construction of professional ethics, and carry out pre job training and post skill improvement training for elderly care workers. Support industry associations and other organizations to identify the professional skill levels of elderly care nurses, baby sitters and caregivers for employees. Employees who participate in vocational training related to elderly care and nursing and whose skill level is recognized as qualified shall be given training fee subsidies or skill upgrading subsidies in accordance with the regulations.
3. Improve the talent incentive and guarantee policy. We will fully implement the salary rating system for elderly care nurses and monitor the market salary level of elderly care nurses. Study and release the market wage guidance price for nurses and other posts. We will strengthen policy support and give appropriate subsidies to qualified elderly care service practitioners such as elderly care nurses. The pension industry will be included in the key support scope of talent introduction in Shanghai. Organize and hold Shanghai endowment nursing vocational skills competition and nursing team skills competition, and carry out evaluation and commendation in accordance with the regulations.
（5） Improve the efficiency of elderly care supervision services
1. Optimize the government service environment. Pilot to promote the establishment of elderly care service institutions "one thing". Regularly publish the list of supporting policies for elderly care and education services, and promote the realization of online application and handling. Support the development of branded and chain pension institutions and facilitate their holding subsidiaries (branches) to enjoy preferential policies across regions in Shanghai. Establish a government service evaluation mechanism for elderly care and education to improve the quality of government service.
2. Improve the comprehensive supervision system of the industry. Implement the measures of Shanghai Municipality on the comprehensive supervision of elderly care service institutions. We will improve the supervision mechanism of care institutions and carry out market regulation and Classified Governance of care services. Establish the credit files of elderly care and education service institutions, implement hierarchical and classified supervision, provide incentives such as tolerance trial and policy and financial support for trustworthy subjects, and punish dishonest subjects according to law and regulations. 3. Prevent and resolve various risks. Strengthen the supervision of large amounts of funds such as pledge deposits and advance receipts collected by elderly care institutions, and standardize the charging behavior of nursery institutions. Formulate and improve regulatory measures for new business forms such as pension communities. Strictly prevent illegal fund-raising activities in the field of "one old and one small" by means of false investment, fraudulent sales and high rebate, so as to protect the legitimate rights and interests of consumers.
3、 Safeguard measures
（1） Establish an overall promotion mechanism
Establish an overall promotion mechanism for the work of "one old and one small" in Shanghai, study and formulate major policies for elderly care, and promote the implementation of key tasks and major projects. Each district has established an overall promotion mechanism for the work of "one old and one small". Relevant municipal departments shall formulate supporting policies and measures according to the division of responsibilities to promote the implementation of various tasks. The municipal development and Reform Commission shall follow up and evaluate with relevant departments.
（2） Increase financial investment
Promote the implementation of the opinions on the construction of elderly care service facilities in Shanghai during the 14th five year plan, improve the support standards for the construction of basic elderly care beds, and provide subsidies for the transformation of existing elderly care institutions, cognitive impairment and nursing elderly care beds, and the construction of various community elderly care service facilities. Study and formulate the supporting policies for the construction of inclusive childcare service facilities during the 14th five year plan, and optimize the reward and subsidy policies for inclusive childcare service institutions. The main body of the operation area is encouraged to provide social support by means of rent reduction, subsidy, etc. Study the linkage mechanism between the demand side and the price reform, and give certain financial subsidies to the difficult objects who are admitted to the basic pension institutions. For children from poor families who enter public kindergartens and inclusive private kindergartens, the tuition fee shall be reduced or exempted. For eligible elderly care facilities projects, actively strive for the support of central funds.
（3） Improve the charging mechanism
Implement the management measures for service charges of elderly care institutions, adhere to classified management, and optimize the price adjustment process of basic elderly care institutions. Improve the charging policies for community elderly care services such as elderly care homes, day care centers and meals for the elderly, and promote professional and sustainable development. Optimize the charging policy of public kindergartens and study reasonable cost sharing rules.
（4） Strengthen the guarantee of land and housing
The construction of elderly care and care service facilities shall be incorporated into the land spatial planning and annual plan at all levels, so as to reasonably ensure the land demand and arrange them in a reasonable location. Strictly implement the construction requirements of "synchronous planning, synchronous design, synchronous construction, synchronous acceptance and synchronous delivery" of public supporting facilities such as elderly care service institutions, kindergartens and nurseries and new houses. For the newly-built elderly care service facilities that meet the catalogue of allocated land, the allocation method shall be adopted for land supply. Reduce the cost of land for elderly care and realize that the overall land price level is equivalent to the benchmark land price of standard factory buildings. （5） Strengthen financial support
Encourage financial institutions to develop characteristic credit for elderly care and provide preferential interest rate support. Give full play to the role of special loans for development finance to support the construction of elderly care service system, and provide large amount of long-term preferential loan support to qualified projects. Innovate credit support methods, explore and promote the pledge loans of accounts receivable and charging rights, and implement the due diligence exemption policy of credit personnel. Give full play to the role of guarantee and credit enhancement institutions to provide credit enhancement support for the financing of elderly care institutions. Support qualified pension institutions to raise funds through listing and bond market. Support insurance institutions to develop liability insurance and operation insurance related to elderly care.
（6） Implement tax preference
For enterprises, institutions and social organizations that provide elderly care services in the community relying on fixed facilities, the income from providing elderly care services shall be exempted from value-added tax. When calculating the taxable income of enterprise income tax, it shall be included in the total income at a reduced rate of 90%. For the real estate and land used to provide elderly care, the preferential policies of deed tax, real estate tax and urban land use tax shall be enjoyed in accordance with the regulations. The water, electricity and gas used by elderly care institutions shall be subject to resident prices. The eligible expenses incurred in the employee's child care center established by the enterprise can be deducted before tax as employee welfare expenses. Social security subsidies shall be given to the elderly care institutions that absorb qualified workers in accordance with the regulations.
（7） Create a good environment
Implement the regulations of Shanghai Municipality on elderly care services, and study and promote the legislation of preschool education and nursery education services in Shanghai. Adhere to the concept of active aging and create conditions for the elderly to actively participate in economic, social and cultural life. Promote the integration of the principle of children's priority into social policies and create a good environment for children's healthy growth. Actively promote the construction of "national key contact city for actively responding to population aging", "national child friendly city" and "national demonstration city for infant care services".
